| Job Information | Job Description: The LPN is responsible for the care of residents and supervision of staff. Essential Job Functions The employee must be able to perform the essential functions of the job with or without reasonable accommodations. Provide a high level of customer service to the residents, family and other employees. Follow established standards of nursing practices and implement facility policies and procedures. Interpret existing policies and procedures to nursing assistants, medication aides, residents, families and physicians. Supervise and evaluate all direct resident care staff and initiate corrective action as necessary. Obtain daily report from nurse that he/she is relieving and record sufficient information to implement appropriate follow-up action if necessary. Conduct resident rounds daily, reporting problems to nursing supervisor and initiating corrective action. Responsible for resident safety, identifying safety hazards and emergency situations and initiates corrective action as able. Follow, supervise and evaluate the implementation of the Resident’s Rights Program. Follow staffing guidelines and make nursing assignments according to qualifications of staff and characteristics of the resident population. Assume responsibility for compliance with federal*, state and local regulations within the assigned unit. Participate in the employee evaluation process by providing information on the clinical performance of nurse aides and medications aides to the DON or RCC. Document resident care provided and resident’s response or lack of response to care provided. Participation in resident care conferences as assigned. Report changes in the residents’ conditions to a physician, Unit Director, RCC or DON and responsible party promptly and take follow-up action as necessary. Follow facility admission, discharge and transfer procedures and coordinate admissions, discharges and transfers with other departments, physicians and ancillary service providers. Counts narcotics, check emergency kit and exchange keys. Prepare and administer medications, enteral nutrition*, parenteral nutrition* and treatments as ordered. Not applicable in an Assisted Living facility. Maintain strict confidentiality of all resident information. Maintain a neat, clean, professional appearance. Assure personal and staff compliance with infection control, personal protection equipment, blood borne pathogens, hazardous material handling and fire safety. Job Requirements: Knowledge, Skills and Abilities A minimum of one year experience as a LPN preferred. The LPN must have excellent communication skills, be able to cope with mental and emotional stress related to the position, function independently, have flexibility and personal integrity. Employee must demonstrate on an ongoing basis the ability to develop and maintain good working relationships with co-workers and department heads. Education/Certification Education: Graduate of an approved nursing program. Licensure: Must have a current and valid License to practice as a practical nurse in the state in which employed.
